How do I enroll?

Answer:

There are 4 easy ways to enroll with FamilyCord:

1. Enroll online
2. Download enrollment forms
3. Enroll by phone – call 800-490-CORD (2673)
4. Enroll by FAX or mail

If your due date is within 6 weeks, we recommend that you enroll online or by phone so that we can expedite the process.
